ABSTRACT

Objective:To explore the key genes and its biological functions of aldosterone producing adenoma (APA) using bioinformatics analysis.Methods:Differentially expressed genes of APA were identified from two training datasets GSE60042 and GSE64957 in GEO database. Function and pathway enrichment analyses for differentially expressed genes were performed and transcriptional regulation network among these genes were determined. Hub genes were extracted by node analysis from the protein-protein interaction (PPI) network. The expression of key genes was verified by a testing dataset GSE8514. Receiver operating characteristic(ROC) curve analysis was applied to assess the diagnostic efficiency of key genes in APA. The biofunction of each key gene were determined by gene set enrichment analysis (GSEA).Results:A total of 68 differentially expressed genes, including 33 up-regulated genes and 35 down-regulated genes, were detected from the training datasets. These genes were mainly enriched in aldosterone biosynthetic process, calcium signaling pathway, serotonin receptor signaling pathway, transcriptional activator activity, and regulation of transcription. JUN and VDR were at the center of the transcriptional factor-gene network. Furthermore, we identified nine Hub genes from the PPI network. In testing dataset, CYP11B2 and VDR showed the higher expression, while JUN, NFKBIZ, EGR3, and KLF6 showed lower expression in APA (all P<0.05), and the value of area under ROC curve analysis was 0.936, 0.833, 0.953, 0.854, 0.868, and 0.929, respectively. GSEA indicated the alter of key genes in APA led to up-regulation of the steroid biosynthesis, cell adhesion molecules, immune cells signaling pathway, and complement and coagulation cascades [all normalized enrichment score (NES)>1.5, P<0.05], but down-regulation of the DNA replication, ribosome, and autophagy (all NES<-1.5, P<0.05). Conclusion:Results of bioinformatics indicate that JUN and VDR are key transcriptional factors, and CYP11B2, NFKBIZ, EGR3, and KLF6 are the key genes for APA, which are involved in the steroid biosynthesis, cell adhesion molecules, immune cells signaling pathway in APA.

ABSTRACT

Of those who have hypertension,several patients are salt sensitive and need to restrict their salt intake. This article elucidates the physiological and pathological mechanisms of salt-sensitive hypertension (SSHT),including the dysfunction of the epithelial sodium channel (ENaC),epithelial damage,and the malfunction of the sodium pump,and relative genetic study,followed by a comparison of various salt-sensitivity examination methods. Another important discussion is about the influence of dietary factors (capsaicin,caffeine, apigenin,taurine,curcumin,menthol,and berberine) on SSHT. This review provides a solid foundation for understanding the biology of SSHT,screening of the salt-sensitive population,and prevention and cure of SSHT through everyday diet.

ABSTRACT

PBL (Problem-based learning) is a kind of teaching method, which is helpful for guiding students to solve practical and complex problems. The compiling of PBL teaching plans is the key to the implementation of PBL teaching. According to the teaching outline of internal medicine, we set the PBL teaching plan, formulated the teaching goal, collected the clinical real cases, finished the compiling of PBL cases of student version and teacher version, and preceded teaching practice accord-ing to the PBL cases. Improving and constructing the compiling framework of internal medicine PBL teaching plan laid the theoretical foundation for carrying out internal medicine PBL teaching.

ABSTRACT

BACKGROUND:At present, a lot of research about culture methods for umbilical cord mesenchymal stem cels, but not for the waste of primary system. OBJECTIVE:To explore the best culture method of human umbilical cord mesenchymal stem celsin vitro. METHODS:Human umbilical cord mesenchymal stem cels were prepared by tissue explants method, recorded as initial culture group. The centrifugal fluid and tissue of the primary culture flask were centrifuged and divided into three groups for secondary culture: tissue group, mixed group and pure liquid group. Cel morphology, time for cel acquisition, and yield of primary cels in the four groups were observed; the cel growth curve was analyzed by MTT assay; and cel cycle and phenotype were detected by flow cytometry. RESULTS AND CONCLUSION: The average time for cel acquisition in the initial culture group, tissue group, mixed group and pure liquid group were (15.00±0.45), (7.0±0.3), (8.00±0.25) and (8.00±0.25) days, respectively. The number of cels at first generation was (4.0±0.5)×105, (9.0±0.55)×105, (15.0±0.2)×105 and (7.0±0.33)×105 markers of the four groups had no significant differences. The human umbilical cord mesenchymal stem cels can be obtained rapidly and largely through the secondary culture to the primary culture system. T75 culture bottle, respectively. Under the inverted microscope, cels in the four groups were fusiform-like adherent cels, which were in paralel or circinate arrangement. Growth curve, proliferative activity, surface markers of the four groups had no significant differences. The human umbilical cord mesenchymal stem cells can be obtained rapidly and largely through the secondary culture to the primary culture system.

[Summary] Clinical studies have confirmed that metabolic surgery is an effective method for the treatment of diabetes ,which can significantly relieve symptoms of type 2 diabetes ,improve various complications and reduce cardiovascular events. Currently ,major limitations of this technique in clinical practice in China include the lack of understanding on surgical benefits ,difficulty in selection of surgery indications and division‐based treatment modalities. Thus ,we should explore a cooperation model of multi‐discipline teams ,understand the mechanisms of type 2 diabetes remission by metabolic surgery and develop physician internist leading standard post‐operation management. These strategies will greatly promote the development of metabolic surgery for type 2 diabetes in China.

ABSTRACT

Objective To explore the role of apelin-13 in regulating stem cell differentiation into vascular net. Meth?ods Mesenchymal stem cells were isolated from human umbilical Wharton’s jelly using tissue adherence method.Their immunophenotypes were detected by flow cytometry . Passage 3 of WJ-MSCs (Wharton’s jelly-mesenchymal stem cells) were inoculated in 4 flasks, denoted as A1, A2, A3, A4 group. TwentyμL of apelin-13 at concentrations of 0, 1×10-6, 10 × 10-6 and 100 ×10-6 mol/L were added to A1, A2, A3 and A4 respectively each day. After being induced for 7 days, cell mor?phology and viability were observed under inverted microscope. Von Willebrand factor (vWF) was examined by immunofluo?rescence and CD31 was identified by flow cytometry. Upon incubating with three dimensional culture medium of hydrogel, those cultured A1, A2, A3 and A4 were renumbered as S1, S2, S3, S4. Again, twentyμL of apelin-13 at concentrations of 0, 1×10-6, 10 ×10-6and 100 ×10-6 mol/L were used to treat S1, S2, S3 and S4 respectively. After 7 days, cell morphology, via?bility and vas-like networks were observed with inverted microscope. Results Our study showed that WJ-MSCs can be in?duced by apelin 13 to differentiate into endothelial cells lineage indicated by positive of vWF staining. Moreover, CD31 expres?sion increases significantly upon apelin-13 addition in a dosage dependent manner. The endothelial cells line formed vas like networks when cultured with three-dimensional medium containing hydrogel. Conclusion This study demonstrated that ape?lin-13 could promote human umbilical cord-MSCs to differentiate into endothelium lineage then to form vascular networks.

ABSTRACT

Objective To construct a recombinant plasmid pUbi-apelin-FLAG-pSV40-EGFP and package with lentivirus to co-express enhanced green fluorescent protein (EGFP) and apelin, and to investigate optimal multiple of infec-tion (MOI) to transfect human umbilical cord mesenchymal stem cells (hUCMSCs) and expression of target gene. Methods The apelin gene was chemically synthesized and amplified by polymerase chain reaction (PCR), and which was inserted into linear plasmid vector. The gene fragment and linear plasmid vector were connected by In-Fusion technology after enzyme di-gestion and transformed into competent DH5αcells. The positive clones of lentiviral expression vector were obtained after screening and followed by sequencing. The lentiviral vector was used to transfect 293T cells and package virus, and then the virus titers were determined. HUCMSCs were transfected with lentivirus vector in vitro via different values of MOI. The trans-fection efficiency was obtained according to the optimal MOI. The expression of target gene was detected by RT-PCR and Western blot assay. Results A 284 bp target gene fragment with the restriction sites was obtained by PCR and connected to the lentiviral vector. The positive clones of lentiviral expression vector were corresponded to the expected result. The lentivi-ral particles were successfully packaged. HUCMSCs could be transfected by the lentivirus vector with high efficiency. The mRNA and protein levels of target gene were stably up-regulated within 2 weeks. Conclusion The lentivirus vector pUbi-apelin-FLAG-pSV40-EGFP can transfect apelin gene into hUCMSCs with high efficiency. The infected cells can express high levels of apelin gene in two weeks.

Due to myocardial necrosis and fibrous scar formation after acute myocardial infarction,patient's ventricular remodeling and cardiac function is reduced,which will cause serious impact on the long-term prognosis of patients with myocardial infarction.As new biological substitute materials,the hydrogels are superior in hydrophilicity and biocompatibility and have been widely used in the research of myocardial infarction's treatment.The hydrogels can be used alone or served as a promising cell/drug delivery vehicle.Many reports show that intramyocardial injection of hydrogels attached with stem cells and drugs can prevent left ventricle (LV) wall thinning,increase arteriole and capillary density,raise restoration ratio of implanted stem cells,facilitate controlled release of bioactive drugs,limit post-ischemic remodeling and preserve LV function in a certain extent.This article reviews injectable hydrogels research progress in the treatment of myocardial infarction.

ABSTRACT

Objective To investigate the optimal condition of lentivirus,which was recombined with marker gene of enhanced green fluorescent protein (Lentivirus-EGFP) transfect human umbilical cord wharton’s jelly-derived mesenchy-mal stem cells (HUWMSCs) and the effect of transfection on the proliferation in HUWMSCs. Methods HUWMSCs were transfected with EGFP by lentivirus vector in vitro via different multiplicity of transfection (MOI) in four different transfec-tion methods (A, B, C and D). The fluorescence expression and the transfection efficiency in different methods were analyzed by both fluorescent microscope and flow cytometry. The proliferation rates of infected HUWMSCs was evaluated by MTT method. Results The transfection efficiency was 10.6%-87.3%after 4 days in all experimental groups, which showed the dose-effect relationship with MOI. Polybrene (5 mg/L) could significantly increase the transfection efficiency (P<0.05). Re-sults of MTT assay showed that there were significant differences in the proliferation rates of infected HUWMSCs between different transfection methods (P < 0.001). There was better cell proliferation in method A (MOI=10) group and method B (MOI=10) group than that of other groups. Conclusion Method B (MOI=10) is the optimal transfection method in this exper-iment. HUWMSCs could be transfected by lentivirus-EGFP with high efficiency and could stably express transfected gene within 2 weeks, which is a safe and effective gene transfer vector.

Abnormal metabolism in vivo of uric acid leads to many diseases.Recent discovery shows that glucose transporter-like protein-9,which belongs to glucose transporter family not only transports glucose,but also plays an important role in the process of uric acid transport,which is also affected by pH,glucose,estrogen,etc.The variation of glucose transporter-like protein-9 results in metabolic diseases.Therefore,the study of glucose transporterlike protein-9 in uric acid transport and related drug will provide new ideas to control the development of hyperuricemia and related cardiovascular disease.

ABSTRACT

Obesity is a major risk factor for the prevalence of cardiovascular diseases ( CV D) in China. The application of imaging techniques is helpful to diagnose the visceral fat obesity. A Large number of studies show a positive relationship between the cardiovascular morbidity and obesity. However, there is not a linear correlation between the prognosis of CVD and the degree of obesity, for example, too high or too low body mass index (BMI) or body fat ratio may increase the risk of CVD mortality. The function and type of adipose tissue determine the impact on cardiovascular system, thus, maintaining an appropriate BMI and body fat ratio is beneficial. Besides weight gain control and prevention of cardiovascular events, one goal of antiobesity is to improve the sickness of adipose tissue.

ABSTRACT

Transient receptor potential vanilloid type-1 channel (TRPV1) is a non-selective cation channel with a preference for calcium ions that is able to sense a vast range of endogenous physical and chemical stimuli and plays an important role in transducing the sensations of noxious heat and pain signaling. Recent studies showed that TRPV1 is widely expressed in different tissues and organs beyond the sensory nerves and has multiple biological effects that are involved in functional regulation in the pancreas, blood vessel, adipose tissue and liver. To further understand the link between TRPV1 and cardiometabolic diseases, we reviewed the role of TRPV1 in hypertension, diabetes, obesity, and dyslipidemia. This review provides new insights into the involvement of TRPV1 channels in the pathogenesis of cardiometabolic disorders and implicates this channel as a potential therapeutic target for the management of cardiometabolic diseases.

The effect of interventional therapy combined with multifactorial intervention on critical limb ischemia in patients with diabetes mellitus was investigated. The patency rate and limb salvage rate were followed up. Interventional therapy is effective in treating diabetic foot with critical limb ischemia. Multifactorial intervention was helpful for reducing amputation.

ABSTRACT

Objective To explore the difference of four calculating methods used to determine the obesity. Methods Two thousand four hundred and forty six (2446) men and three hundred and seventy nine (379) women were measured height and body mass, Standard body mass, BMI, body fat ratio and obesity index(OI) were calculated with formula. According to the BMI≥ 25 kg/m2 , BMI ≥ 27 kg/m2 and BMI≥28 kg/m2, the obese adults were 1419,680 and 435 respectively;there were 649 adults that their body mass was over 20%standard body mass; there were 639 adults that their body fat ratio was over 25%(male)and 33%(female). Results For obesity determination, the adults who were 20%overweight and over standard body fat ratio were significantly different from those whose BMI were over 25 kg/m2 ( P<0. 05 ) ;Those who were 20%overweight and over standard body fat ratio were not significantly different from those whose BMI were over 27 kg/m2 ( P>0.05 ) ; Those who were 20%overweight and over standard body fat ratio were significantly different from those whose BMI were over 28 kg/m2 ( P<0. 05 ). Conclusion Determining obesity with BMI≥27 kg/m2 is feasible and rational.

Objective:To analyse the diagnostic of multislice CT(MSCT)for acute appendicitis.Methods:Sixty-seven patients with acute appendicitis confirmed by surgery and histopathology were analyzed retrospectively.The sizes of appendixes,appendolith,periappendiceal changes were observed and noted.Results:Sixty-four of the sixty-seven patients were correctly diagnosed as acute appendicitis.The direct CT signs of acute appendicitis were an enlarged appendix(82.08%).Indirect signs includedappeniceal perityphlitis(73.13%) and preiappendicular abscess(11.94%);and three cases were not observed with CT.Conclusion:Acute appendicitis has a variety of CT signs which is helpful to improve the diagnosis.

ABSTRACT

Objective To study the long-term effect of administration(6 months) with transient receptor potential vanilloid 1(TRPV1) agonist capsaicin on contractile reactivity of thoracic aorta in C57BL/6J mice.Methods Tow-month-old male C57BL/6J mice were received normal diet group(n=12) or capsaicin group(normal diet plus capsaicin,n=12).Tail-cuff systolic blood pressure(SBP) was examined at the baseline and at the end of the intervention.After 6-month treatment period,carotid artery blood pressure and heart rate were determined by catheterization,and the aortic contractile response was examined using isometric myograph(Danish Myotech Technology,Denmark).Plasma levels of renin,angiotensin Ⅱ(Ang Ⅱ) and aldosterone were determined.Vascular smooth muscle cells(VSMC) were obtained from thoracic aorta of mice and cultured.Angiotensin Ⅱ type 1 receptor(AT1R) protein expression was detected by western blot.Calcium imaging was detected in cultured VSMC using the fluorescent dye technique.Results Systolic blood pressure,invasive carotid artery blood pressure and heart rate have no difference between two groups.No differences was found in PE-induced contraction response in thoracic aorta;while Ang Ⅱ induced contractility of aortic ring was lower in mice with capsaicin than control group [capsaicin:(37.5?1.6)% vs(59.8?1.4)%,P

ABSTRACT

Objective To study the target organ damage in patients of hypertension with metabolic disorder. Methods 1033 patients were divided into five groups: hypertension without complications(102 cases), hypertension with dyslipidemia (117 cases), hypertension with abdominal obesity(119 cases), hypertension with type 2 diabetes mellitus(135 cases), hypertension with metabolic syndrome (560 cases). The structure and the function of heart and blood vessel were examined by color Doppler. Urinary microalbuminuria was determined. Results (1)Only 9.9% patients of hypertension were without metabolic disorder, 90.1% hypertensives complicated with the metabolic disorder; (2)Compared with hypertension with dyslipidemia, hypertension with type 2 diabetes mellitus (P90%) complicated with the metabolic disturbance. Hypertension complicated with abdominal obesity accentuate left ventricular hypertrophy. Hypertension with diabetes mellitus and metabolic syndrome accentuated the vascular and renal lesion.
